How To Verify Benefits: Questions For Your Insurance Provider

Ask about your plan's coverage for the following three nutrition-related CPT codes (Current Procedural Terminology)- these are codes that indicate services are provided by a Registered Dietitian Nutritionist.

  • 97802 (Initial Assessment)

  • 97803 (Follow-up Session)

  • 99404 (Prevention & Wellness)

​

Potential ICD 10 diagnosis codes to combine with these CPT codes are:

​

  • Z71.3 for Blue Cross/Blue Shield and Cigna plans

  • E66.3 for Aetna and United Health Care plans, if you have a BMI >25 (Click HERE for a quick calculation)

  • Medicare and Medicare supplemental plans restrict their coverage to diagnoses of diabetes and non-dialysis kidney disease ONLYWeight loss is not a covered diagnosis

Note: In our discovery call, I can provide you with potential diagnosis codes based on information discussed.

Important questions to ask:

​

  • Is tele-health provided by a dietitian a covered benefit at this time?  

  • How many visits per calendar year are permitted?

  • Do you have a deductible to meet? 

  • Is a co-payment required?

  • Is a physician referral required?

  • Is coverage limited to a specific medical diagnoses?

    • As an example, Medicare benefits for nutrition counseling are limited to those with a diagnosis of diabetes, or non-dialysis kidney disease ONLY. Weight loss is not a covered diagnosis.

In our first session, we will discuss your eating habits, your health concerns, and identify barriers. We will talk about what you enjoy eating and how we can reinvent your favorites to support your goals.

 

Who shops? Who cooks? Where do you eat out? All these come into play when putting together a plan tailored to your life. At the end of each session, you will leave with a list of achievable goals to work on between visits.

Follow-up Visits

In our follow-up sessions, we will check in on your progress and continue to build sustainable habits. Planning meals not your strength?  We will find workable ideas.  Don’t cook?  Let’s get you started.  I have easy tips and strategies to get a meal on your table in 30 minutes or less.

Setting Up for Success:  $320 

This package includes 1 initial session plus 2 follow-up sessions. Advance purchase includes between session support to help you stay accountable and a 2 month subscription to Living Plate Rx Meal Planners.

Building On Your Foundation: $240 

This package of 3 follow-up sessions is unlocked after an initial assessment with Ellen Petrosino. Advance purchase includes between session support to help you stay accountable, and a 2 month subscription to Living Plate Rx Meal Planners.

Support and Accountability: $200 

This package includes 5 telehealth video check-in sessions (15 minutes). Each week we will celebrate successes, and identify new connections you have made. We also discuss challenges that arose and together we create targeted strategies to help you navigate these personal roadblocks in the future.
